Accel+[ / ] keybindings should work for back / forward command on *nix/mac, like
in the browser itself.

One nit: I know the entity capitalization is consistent with that used in
browser.dtd, but it's completely inconsistent with pretty much every other
entity name.  Switch it from "commandKey" to "commandkey" and r=me.

I'm busy hacking up help.xul right now for bug 260058 and bug 253334, but I'll
save a diff of my changes and check in your (revised) patch in a little bit.
