I am debugging an application and noticed more than one cookie for my domain with the same name (but different values). This should not be possible and is likely the root cause for many of your "cookies don't work" bugs.
Summary: Two cookies with same name for same browser should not be allowed → Two cookies with same name for same domain should not be allowed

And how were you seeing this? By reading your cookies.txt file? Or with server side script?

don't forget that cookies with different paths are different cookies (according to the spec)
