27964 - split out table in boilerplate page

Some of the boilerplate in the abovementioned page are generically filled out
for any contributors, and some are specifically filled out for Netscape
contributors.  The existing table should be segregated along these lines, which
also means adding another version or two of the boilerplate.  Most
interestingly, MPL-with-GPL dual-license that is not specifically filled out for
Netscape.

This bug has not been touched for more than nine months. In most cases, that 
means it has "slipped through the net". Please could the owner take a moment to 
add a comment to the bug with current status, and/or close it.
