Approving some things is creating two entries in the approval log
Maybe its one per app.  Except you can't approve one app without approving them all.
It also happends when you Deny an extension or theme.
Is it creating one entry for every application that it was submitted for?  Like
if I deny an extension that was for both TB and FX, does it create one entry for
each of them?

Remora doesn't have the same type of approval log (yet?) but when it does, versions are not stored the same way and I'm extremely confident that this issue would not come up.
