Except for the bugzilla product, it seems that the bugzilla helper, in its
second paragraph, says "Consider using the Bugzilla Helper instead of this form"

It isn't on

not a global thing.  Unless you overrode it there.

As far as I can see, this is occuring on all major bugzilla starting pages
linked from
https://bugzilla.mozilla.org/enter_bug.cgi

All clicking the link on any of those pages causes the same page content to be
reloaded, but with the string format=guided& inserted into the request

Sometime in the 2.19 timeframe, Bugzilla stopped passing the format parameter to the enter_bug template, so when it shells out to the user-message template that is included at the top, the conditionals for which format it was being loaded from weren't working.  That's been added back, and it works now.
