DOM inspector is listed as an installed extension but there is no menu item
under 'Tools' for it. CTRL-SHIFT-I doesn't have any effect either.

Um, kinda old build you're using. In Firefox 1.0 and trunk builds DOM Inspector
is not listed in Extensions manager, but is installed if you for example use the
Custom install option in Windows installer.
