The config settings 'mousewheel.with*key.action' do not work on the mac.
According to http://preferential.mozdev.org/preferences.html integer settings
from 0 to 3 have different effects.
These all work as expected in the 'withnokey' option, but any value in any of
the other options has no effect: the mousewheel with that modifier simply
scrolls -- as if the value were 0.
I assume alt -> option/alt, control is just control, and there is no option for
the mac's command key.
