If a select list has javascript that makes the next field on the form hidden
when a specific value is selected, if the user selects that value and then hits
tab, the user is brought to the hidden text box.  In that text box, the user can
type.  

In addition, a text box that is made visible in this same way is ignored in the
tab order.

Reproducible: Always

Steps to Reproduce:
1. Go the http://dev.ddig.com/valcpa/mozillabug.html
2. Change the Home Country or U.S. State: field to Other -- Not Listed
3. Using the keyboard, change it to something else, such as Vermont.  
4. Press the tab key

Actual Results:  
The user is able to type in the hidden text box

Expected Results:  
The user should have been brought to the next visible field

In addition, if you repeat the steps to reproduce, but switch to Other from
another value, the browser skips the newly visible field.
