1. fix mis-transform between pdf and html Bob Clary transformed(already reported
him by email.)
2. replace physical tags(i,tt,b)

in the example below, I added some classes(see
http://moz.rsz.jp/devmo/js/specs/ecma.txt) to assimilate to pdf.
if need not assimilate to pdf, those classes can be removed.

residual problems: in HTML4.01, attribute of ID should be uppercase.

example files (original html is xhtml 1.0 Transitional):

actually, I've told this is WONTFIX on 2005/Feb or Mar.
just i(or someone) didn't change this.

and i don't want receive notify about such DEAD bugs ;-)
