repeated use of the "Save As..." or "Save Image As..." dialogs in both firefox &
thunderbird populate the "recent" items folder of the Save Dialog incorrectly. 
the second time a save is performed, folder names will appear twice.

Reproducible: Always

Steps to Reproduce:
1. in some web page (or mail message) with multiple images (attachments), save
   one, then the next.  ("Save Page As..." will cause this behavior, too.)
2. choose a new location where to save the item
3. for the 2nd save, note the contents of the recent items menu in the save dialog

Actual Results:  
menu was populated with duplicate entries from the folders i saved to.

Expected Results:  
menu should not duplicate entries

this has the negative side-effect of reducing the useful lifetime of recent
items in this folder, because the recent items size is limited, and
double-naming in this part of the dialog causes older items to be LIFOed
prematurely.
