Download options (Options->Download->Filetype settings) won't be obeyed when you
click on an external link while firefox isn't running yet.

I've, e.g., set .pdf-files to be downloaded and opened externally by Adobe
Acrobat Reader in the menu Options->Downloads->Filetypes (Open with: AcroExch).
This works well with links in webpages displayed in Firefox, i.e. when Firefox
is already running.

But when Firefox isn't started yet and I click on a URL pointing to a pdf-file,
Firefox starts up but loads the Acrobat plugin to present the pdf in the Firefox
window internally instead of starting Acrobat Reader displaying the pdf
externally as set in the options.
Clickling on the same link with Firefox already running will start Acrobat
Reader as expected.

Steps to Reproduce:
1. Set your download options to always open pdf-files externally via AcroExch
2. Send yourself a URL to a pdf-file by mail
3. Close all instances of Firefox
4. Click on the link in your mail app

Actual Results:  
Firefox starts, but pdf is shown in Firefox itself

Expected Results:  
Firefox starts, downloads the linked pdf-file and then starts Acrobat Reader
with the downloaded pdf.

I'm running Mozilla/5.0 (Windows; U; Windows NT 5.0; de-DE; rv:1.7.6)
Gecko/20050321 Firefox/1.0.2
Seems not to be reproducible on other machines.

Maybe it has something to do with the emailer use: Thunderbird.

With other mail clients on different machines there seems to be no problem.

I'll look into it further.
