In "Page Setup" - "Margins & Header/Footer", the options are relevant to a web
browser, not a mail client. They are Title, Date, URL, and page numbers. When
Title is selected, the Subject: is printed, but there is no way to select a mail
header like To:, From:, etc.

Confirming. I couldn't find a dupe of this and it seems relevant. Possibly
related to bug 21432.

Thanks for bringing this up!  We really need to watch the Firefox terms that creep in.

Perhaps an easy rename of Title to Subject can be done.  Date seems relevant to me but maybe that should be more specific about Message Date; not sure that would really clarify.

We should make a list of some header to include.

To, Cc, From, Reply-To, Priority

I think most of this makes sense to get in the defaut and lots of the printing features from nic-nac also look good to me.
