What version of Bugzilla is this in? Have you modified your local installation?
When did this start happening?

I have installed version 2.18 and patched it with the Custom Fields mod from
https://bugzilla.mozilla.org/show_bug.cgi?id=91037.
Bugzilla is running on a Windows 2003 Server using Apache as the web server
and MySQL as the database.
From what I can remember it has been happening from Day 1 (Before mod)

> Ever since the Custom Fields mod
There is no custom fields mod which is officially supported, and it's quite
likely it's the reason for your problems. This makes it a support issue rather
than a bug, so we'd be better off discussing this on the mozilla-webtools list.
