create a new bug by entering all the data and clicking the Commit button. Then 
clicking the Refresh memu button with create a duplicate record. Keep clicking 
the Refresh button will keep creating the records. 

Reproducible: Always

Steps to Reproduce:
1. Click New on the main page.
2. enter data and click the Commit button
3. Click the Internet Explorer Browser's refresh button

Actual Results:  
Recreate the record

Expected Results:  
Goes back to the newly created record
