URL and Title page headers do not show up on print preview or printed output

I noticed URL and Title page headers were missing on printed web pages, checked
my config, title is in configured upper left, URL in upper right, but they do
not appear.  I changed URL to Date in upper right and it then appeared in
preview and on print, changed back to URL and it disappeared.  Same behavior
with Title.

Reproducible: Always

Steps to Reproduce:
1. View a web page
2. Go to print preview, note if Title and URL headers are visible
3. Press Page Setup, go to Margins/Header&Footer, change various settings back
and forth, Title and URL will not display, others will.

Actual Results:  
Title and URL will not display in preview or print

Expected Results:  
Title and URL should display in preview or print
Works for me.
