Out of curiosity, what additional information would you need in order to know
what to fix?

This is a highly annoying bug, as not only will Firefox not RMB open new
windows, but new windows are NEVER opened. Even when you have clicked a button
to open one up. If there's no RMB->Open in new tab option you're screwed.

This bug first popped up (no pun intended) in 1.0.3, and is HIGHLY annoying.

I am considering going back to 1.0.2, and can NOT recommend 1.0.4 to any new
users, as this bug removes a fair amount of functionality.

I'm more than willing to help provide some additional info, but need to know
what you need.
