The feature "use small icons" in toolbar customization is both ambiguous and
obsolete, and does not take into consideration smaller status-bar icon sizes and
the customization of individual toolbar sizes.
There exists 4 standard icon sizes used in Firefox. 48x48, 32x32, 20x20 and
16x16. 48x48 are the default toolbar button sizes, 32x32 are the "use small
icons" sizes, 20x20 is found in throbber and status-bar icon sizes, and 16x16
are found in the menu-bar, url-control, search-control, tab-bar, and sometimes
in the status-bar. Currently, there is no way to define 20x20 and 16x16 when
customizing a toolbar.
I propose to describe these sizes as "Large", "Standard", "Small" and "Tiny"
respectively. Noting that Large is the default size and Standard is the size
used by "use small icons".
Including 20x20 "Small" and 16x16 "Tiny" would set a standard with Theme makers
to include these sizes with their themes. This would allow more icons to fit in
the menu-bar and status-bar, and would allow for even smaller toolbars for those
of us with huge monitors and good eye sight. I know the difference between
20x20 and 16x16 is insignificant, but these are still standard sizes that should
be supported less we kill 20x20 in favor of 16x16.
Additionally, the ability to specify individual toolbar sizes should be added as
a standard feature in Firefox. The ability to drag toolbar icons into ANY
toolbar (including menu-bar, tab-bar and status-bar) should also be added. These
features are currently available through Toolbar Enhancements.

This is a follow up comment to confirm that icon size specifications in Firefox
are still ambiguous. It is still impossible to fully customize a toolbar with
icons of matching sizes because extension writers have no set standards to work
with. 20x20 and 16x16 toolbar icons do exist, but there is no way to specify
"Small[er]" and "Tiny" inside the toolbar editor dialog, making access to these
icon sizes limited to the menu-bar and status-bar.
Please consider a universal method of sizing and placing tool icons in all
available tool/menu/status bars, utilizing all 4 icon sizes.

Was just about to make my own bug on this when I came across this one, I was going to post:

"In Customize Toolbar, the tick box "Use Small Icons" is unchecked by default.
However on a default profile the menu bar (File, Edit, etc..) and the bookmark toolbar both use small icons. To make the menu bar use big icons you need to tick the "Use Small Icons" then untick "Use Small Icons", the behavior admittedly stems from my own reported bug 417471 which I think is still valid, it just causes this behavior to become even more peculiar.
What's even more peculiar about this is that it has no affect on the bookmark toolbar at all.
The least impact solution I can think of:
1) The checkbox by default has something like a greyed-out tick so it's clear that it's a mixed answer (or some other obvious solution to it being a mixed answer)
2) Unticking the box makes the bookmark toolbar buttons big as well as the menu bar.
This way while the behavior is still a little odd, at least it sort of makes sense and there no serious UI changes and no string changes."

Given this will likely miss Firefox 3, I'm going to try and think up a minor UI change, I don't think the suggestion in comment 0 is appropriate for the current set-up.
Looking for suggestions on a good design, while planning this I'm learning to code for Firefox, but given I'm only at the point where I've learnt how to download the source code so not going to pick this bug up at least yet.

Ignore my previous 2 posts for now, I hear we're getting a beta5 so I'm going to try push for this to be fixed, so here is the current behaviour (menu bar is the one with File, Edit etc...):
1) "Restore Default Set" button
* Menu bar = small
* Nav bar = big
* Bookmark toolbar = small
2) Tick box "Use Small Icons"
* Menu bar = small
* Nav bar = small
* Bookmark toolbar = small
3) Untick box "Use Small Icons"
* Meunbar = big
* Nav bar = big
* Bookmark toolbar = small
This is 3 options, spread across a checkbox and a button, the 3rd option of which doesn't seem to make a whole lot of sense anyway and can only be selected by going through the 2nd option, so hardly intuitive to get at.
Either unticking the box for "Use Small Icons" should make everything big, or the interface should just be designed per-bar.

Well that was a bust, looks like nothing new is going to land for Fx3. Will see about raising this on some discussion group and see if we can't get the whole thing redesigned for Firefox 4

It's been noted that work around this is going on on SeaMonkey, see: bug 394288 and bug 428216
Hopefully it'll be possible to power the work over :)

Firefox is getting a big revamp of the customization UI in bug 770135, so it's unlikely that this bug will still apply afterwards, and if it does a new bug can be filed as the discussion and considerations here will be different.
