No difference in http://.  After closing Firefox browser and using another
browser, IE6 or Netscape8; and then returning to reopen Firefox, the toolbars
have returned to the default settings and keeping my customizations.

Steps to Reproduce:
1.Open Firefox make Firefox my default browser
2.Customize toolbar
3.close Firefox
4.Open IE 6
5.Close IE 6
6.Open Firefox
7.Customize toobar I had just customized ten minutes ago. 
8.Close Firefox
9.Open Netscape 8
10. Close Netscape 8 
11.Open Firefox
12.Customize toolbar I just customized 20 minutes ago
13. Close Firefox

Actual Results:

Updated versions of Firefox (Deer Park) are released every day. Perhaps this
problem has already been solved. Please test this with a nightly build available
at http://www.mozilla.org/developer (bottom of page).
I can confirm this. I installed Mozilla/5.0 (Windows; U; Windows NT 5.0; en-US;
rv:1.8b4) Gecko/20050831 Firefox/1.0+ zip build and lost my custom toolbar. I
narrowed it down to "MR Tech Local Install 3.0.1" extension. Once i disabled it
i got my custom toolbar back.
Another note is that I was using Aug 19 nightly branch build that was fine with
that extension enabled. So it must have broke sometime between the two nightly
branch builds.

From Conversation with Gena on IRC. You only need to do your customization, then
open a new window to see that the default toolbar set has returned.
Ok Here is more info. It seems that I had some options changed in an older build. 

The checkboxes checked are:
1. Hide 'Copy UserAgent/Build ID' menu option
2. Hide 'My Config' menu option
3. Hide 'Open Profile' menu option
4. Hide 'Local Install Settings' from Tools Menu

Once I unchecked them and restarted the custom toolbar was back. Also
re-checking those 4 checkboxes now seems to work.
I'm not 100% but this might be due to an issue with the about: menu options in
the my browser xul overly being slightly malformed in 3.0.1.  This has been
resolved with the upcoming 3.1.0 release, very stable test build is available
here: http://www.mrtech.com/extensions/local_install_dev.xpi

Seems to be a branch issue, here are others with issues and don't have my
extension installed:

I also experience this issue since the early Deer Park Branch builds, and it is
also present in the 1.5 Beta.  In my case, any of the following extentions, if
installed and enabled result in a customization reset (disabling the extension
restores the customized states):

1. Web Developer 0.9.3
2. Header Monitor 0.3.2
3. IDN Info 0.6.4

Thus, as noted by Mel, something checked into the Branch b0rk3d something.
Old report, WFM; from the comments it seems like that it was a number of different causes (e.g. add-on compat) and it's likely that these have all been handled on a case-by-case in different bugs or by the authors of these add-ons.
