I've been seeing this for some time, seemingly very randomly, with the "xxx
wants to set a cookie" alerts--but never reproducibly.  

Now I've found a login (HTTPAuth) that displays, reproducibly, as a
old-fashioned modal dialogue centered on the browser window instead of as a
sheet like all other  HTTPAuth logins and alerts are.

1. Go to http://nagios.mozilla.org/
2. Click "Downtime" in the left-hand column
3. Observe a login dialogue rather than a login sheet
Another one is the "Your message hasn't been sent; do you really want to discard
it" JS dialogue at Gmail.
I wasn't able to repro either of those. I suspect this was fixed along with bug
301432.

I saw this within the last week or so for a cookie dialogue, but those are never
reproducible :-(  I'll reopen if I ever find another consistent case.
We will do this if we can't find the parent window for the request, so this is
"normal" in some situations.
