When building a Mac menu, check for "hidden='true'" and don't display that 
menu item. Thus the XUL/JS usage of "hidden", which triggers the CSS rule
"display: none" on Windows and UNIX, will work on all platforms.

verified fixed with trunk and branch. A top-level <menu> with 'hidden="true"' is
not visible and setAttribute/removeAttribute will show/hide that <menu>.
