"Close Window" sometimes does not properly revert to Command-W

Make sure you have one browser window open with multiple tabs, and either the downloads or preferences window open just behind it. Close the browser window with Command-Shift-W, and then attempt to close the other window with Command-W. Note how the command key for the menu item hasn't changed back as it would if the windows were simply swapped. (Beep-beep.)

Fixed on trunk and branch by updating the menu items when window layering changes (using the existing code from which we updated the bookmarks menu). I also tweaked things to avoid redundant updates (when window A loses key, and window B gains it).
