All extensions enabled after software update

After updating from the 7/25 build to the 7/26 build, I found that all of my
extensions had magically become enabled.  This even included extensions that are
not compatible with 1.0+!

I suspect this has something to do with the extension manager and possibly
extensions.ini being deleted when compatibility.ini changes.  So, I'm assigning
this to bsmedberg ;-)
I can confirm that my one and only incompatible extension is now showing as
enabled, and is, in fact, enabled.

It also appears the Extensions window has shrunk the size of the items listed
considerably (I can't really see what changed, though I guess it must be all
that whitespace got reduced).

This was caused by bug 264750 and should only affect nightly users when the
app.extensions.version has not changed so a downgrade / upgrade will put
everything back to normal. Patch coming up.

This migrates the disabled attribute to userDisabled or to appDisabled if it is
an incompatible extension

Note: this can be removed prior to the 1.1 release at the same time as the
app.extensions.version is bumped since at that time the compatibility check will
occur again. Also, there is migration code that already does essentially the
same thing for migrating from the 1.0.x extensions datasource.
