Save Page As feature doesn't save pages as file name, but as undefined.

Reproducible: Always

Steps to Reproduce:
1. Visit any page.
2. Right click --> Save Page As or File --> Save Page As
3. Notice undefined.html, ect.

Actual Results:  
Page saves incorrectly

Expected Results:  
Page save as file name

The problem doesn't occur in Safe Mode, which would suggest the bug is in an
extension and not Firefox.  If was to guess which extension this is from I would
think Mozilla Archive Format, since it does modify Save as type option in the
affected dialog.
