regression from bug 202014 "OS X: add header/footer configuration for printing"

As reported in bug 202014 comment 51 and below, there's a problem with the
popups in the print dialog : the "(Blank)" entries in the popup appear to be
selected, although they're not really used. See attachment 192009 [details] . This only
seems to appear on Mac OS X 10.2.8, not on Mac OS X 10.4, according to Simon.

I think that the problem is here :

 The "(Blank)" entries of the popup have a 'checked' property set that is
unneccessary. Apparently, this gets cleared somehow in 10.4, but not on 10.2.8

It doesn't seem to appear on 10.3.9 either (SeaMonkey 8/8 trunk).
