Forms in the latest version are not displayed in thier entirety.  The icon for the 
action needed to proceed is lost but a hot spot maybe can be found.
It is impossible to do business with this browser after all these months.

Have you tried this is safe-mode? It's likely one of your extensions could be

The submit button in the PCTools URL you provided is served from akamai.net
rather than from pctools.com, so there's at least two ways you could have wound
up hiding it: in Options/Web Features checking the "for the originating site
only" box, or by using either the built-in image blocking or an extension like
AdBlock to block images from akamai.
