www.quickarrow.com is a corporate time and expense system.   The time entry
screen displays a note, but Firefox does not add or change notes.  

On the same site, the exense form fails to paint correctly.  It hangs.

Steps to Reproduce:
1. Sign into Quick Arrow
2. Select time or expense
3.

Actual Results:  
Time card displays correctly.   The notes feature fails.
Expense form - 
1. Total column on wrong side of form.  Total amount is X.
2. Columns not sized correctly
3. Pull downs do not work
4. Notes do not work

Expected Results:  
Input screens should work - Notes and pull downs.
