We would like a configurable option in XRE that will force the application to be
the front window on OSX at startup.  Attached (after this post) is a patch that
does this, though we did not go to the extent of making it configurable.

It's not configurable either, but bug 345057 brings the application to the front at launch time, even after xpcom restart, if it's supposed to be in the foreground.
