Go to manage bookmarks.  Right click a bookmark or folder and delete the name
from properties dialog.  Click OK.  The blank named bookmark is admitted and is
visible everywhere.

The names should not be allowed to be left blank.  while editing disable OK in
such a case.
why shouldn't it be allowed ?
Ok, I'm to mark a series of bookmarks with no names and showing only icons. does
that make some sense?  So it is better to prevent such things happening
accidentally.  This is a common logic used almost everywhere else.  Give it a
thought.
