If you have one item in DM and press [clean up] you end up with an empty DM and
it requires an extra click to close it.
I'd prefer it to automatically close if the last item is cleared.

preferably, show the list is cleared and close the window after a second or so.
I would vote against this. It saves a click, but it's totally unexpected for the
user.

I would definitely prefer the downloads window to close when it's emptied (either by removing individually or the cleanup button), even as an option.
I vote for this. Maybe there can be an extra button labeled "Clean and close". This will solve our need, and also be user friendly, as Jo Hermans said above.
I can't see this being added really. It seems to be to be completely unexpected by the user and adding an about:config pref for this would seem to be to be overkill.

No, I wouldn't like to see this optinal either.
The fact that you're not used to it or wouldn't expect it doesn't mean it's wrong idea though.
As long as there's a certain delay it should be fairly obvious to the user what is happening.
It's a matter of getting used to.
Another suggestions is to add a checkbox in the download options just bellow the checkbox labeled "Close it when all downloads are finished". This new checkbox would be labeled "Close it when the list is cleaned". 

I think this would solve this issue in a good manner, making everyone happy :)
Sorry, but I think this breaks user expectations too much.
