In FF 1.5 (Beta 1), there is an checkbox in tab preferences labeled "select new
tabs openend from links".  When checked, this appears to bring new tabs to the
foreground.  At least, this seems to be the behavior... except bookmarks. 
Middle-clicking (or Ctrl+click) a bookmark always opens the new tab in the
background.

I would think that the "select new tabs openend from links" would apply to
bookmarks as well.
links is "links in webpages"
And bookmarks have their own hidden pref, which you must have toggled at some
time in the past: browser.tabs.loadBookmarksInBackground defaults to false.
No, my preferences page does not say "link in webpages".

Also, there may be a hidden setting (probably set via the tab preference
extension before I installed 1.5 beta 1).  But if users are going to be given
the option, it should be consistent.  Either add another checkbox for bookmarks,
or have bookmarks act the same as all other links.
Well, no, they shouldn't be the same: the defaults are chosen to match the most
common uses, opening several links in new tabs for later reading, but opening a
bookmark in a new tab because you want to see it now without clobbering the
current tab. However, asking for UI for the pref is a valid request, it just
happens to be a duplicate.
