I'm running FireFox 1.0.7 under W2K SP4 (all Windows updates applied).
Under Tools / Options / Downloads / File Types I get no automatic actions for
any filetype listed. Both 'Change Action' and 'Remove' buttons are greyout. I
have tried reinstalling but get the same results. Documentation appears to
suggest I should have a modifiable list here. How do I create these actions?

1. Install FireFox 1.0.7
2. Click on Tools / Options / Downloads

Actual Results:
No automatic actions listed for individual file types.
Expected Results:
A list of actions for different file types that should be modifiable.
Maybe related to bug 308204?
Yes, it looks like a duplicate of bug 308204. I can confirm that I installed version 1.0.7 under W2K and no previous versions were installed before that.
