currently it appears that header and footers are placed relative to the physical edge of the output page (see preview)
this is not very helpful as often the printed text is only partially printed
it would be much more useful if the header and footers where relative to the logical page margin (rather than the physical page margin). The logical page margin is defined via the header/footers tab in page setup
this allows users to tweek the page margin for their particular printer.
it also looks better - almost all printed pages align page numbers and
chapter titles with the left and right logical page margins

PS not sure if I should file a seperate bug but is it possible to suppress or move offscreen the irritating popup window when switching to preview mode?

MozillaAS v1.7.x is not supported anymore.
Can you reproduce with SeaMonkey v1.1.9 ?
