I´ve a bookmark bar with many items in it. Some of the icons for different pages are set to the icon of the first element in the bookmarks toolbar. The bookmarks are attached.

if a site says "shortcut icon" it works in the location bar, and in the
"bookmarks toolbar folder", but if it says "Shortcut Icon" it works in the
location bar, but not in the "bookmarks toolbar folder".

Test with www.uccu.com, which has <link rel="Shortcut Icon" href="favicon.ico"
type="image/x-icon">

comment 3 is bug 258867, nothing to do with case sensitivity.
