A browser window opens with no bookmarks in the bar, and the home page does not display. The address bar does not function. The only way to navigate is by clicking the doodad at the upper right to go to the Firefox home page. Also, the green "updating" symbol shows up in the menu bar and does not disappear.
Reproducible: Sometimes
Steps to Reproduce:
Double-click an Internet shortcut TWICE. The Internet shortcut must be configured to open Firefox.
Actual Results:
Here's what I think happens: Two browser windows open. One is functional and shows the opened web page. The other is the dead window as described above.
Expected Results:
Two copies of the web page should have opened, both functional (unless the browser is configured to open all web pages in the same tab and window).
I believe a sentence to the effect of this bug has appeared in at least one magazine publication. I have not yet downloaded Firefox 1.5.
Just tried v1.5. After doing the steps I listed, I found a new behaviour: instead of the 'dead' window I described before, I get a completely blank, white window.

Please retry with Firefox 1.5 installed in a new, clean directory and in its safemode to rule out extension problems.

If it makes no difference, try a new profile.
New profile: close Firefox, restart your computer, click on the Windows Start
button and type %appdata% in your Run box. Rename the folder Mozilla to
_Mozilla and start Firefox again.

So far I've only reproduced the bug when Firefox 1.5 is not yet open and I have just logged into a Windows session. I was able to reproduce the fault after removing an extension Firefox marked as out of date and disabled. I have not tried your second suggestion yet. I believe the bug only manifests when two copies of Firefox are opened in quick succession.
All Firefox windows run in one process, so perhaps this is a race condition for the acquisition of a resource on startup? Maybe someone's been into the FindWindow API as a mechanism for determining if another copy of an executable is already running....
The blank window initially appears in the upper left corner and has dimensions 0 by 0. Its caption says just "Mozilla Firefox," but no document.
Upgrading severity and qualifying branch. This bug is strange and relatively easy to reproduce.

This bug was reported using a version of Firefox that security and stability updates are no longer provided for. All users are strongly encouraged to upgrade to Firefox 3 by selecting 'Check for Updates' in the Help menu or by going to http://www.mozilla.com/en-US/firefox/firefox.html
If you can no longer reproduce this bug using the latest Firefox 3.0.x version, please change the status of this bug to 'RESOLVED' 'WORKSFORME'.
