[RFE] Cookie manager needs "remove and block" button

Details
The cookie manager should allow you to automatically add a site to the block
list when you remove the cookie. Maybe this should be the default behaviour.
The reason is that in most cases, people are going to go through the list of
cookies. For the cookies in the list that they want to remove, they aren't
likely to want to have the cookie re-appear later.

An excellent suggestion. I like it so much that I just implemented it. Will
check it in as soon as the tree opens today.
