I have a livemark that sucessfully loaded last time I ran Firefox, but the host is inaccessible this time. The result is the menu contains all the old items (that's a nice change of behaviour!) but also contains the "Live bookmark loading..." item at the bottom.

I don't think it's right to show the items *and* the loading item. If the items are there, the loading placeholder is not necessary.

If you happen to open one while it's reloading, why would you want to not know that you are seconds away from possibly seeing something new? And if you force-reload one (once there's a Places way to do so), why would you want to have absolutely no way of knowing whether or not it's actually happening?
The correct place for that sort of notification is the folder icon. The dummy item only exist(ed) because there would otherwise be no items in the menu, AIUI. It would be fine to include it if it was the first load, but after that there are real items there - mixing a placeholder with them is really silly.

For good or ill, the new livemark service does indeed only insert the livemark-loading item if the feed hasn't been loaded before (though bug 386603 wishes it wouldn't).

Bug 451915 - move Firefox/Places bugs to Firefox/Bookmarks and History. Remove all bugspam from this move by filtering for the string "places-to-b-and-h".

In Thunderbird 3.0b, you do that as follows:
Tools | Message Filters
Make sure the correct account is selected. Click "New"
Conditions: Body   contains   places-to-b-and-h
Change the action to "Delete Message".
Select "Manually Run" from the dropdown at the top.
Click OK.

Select the filter in the list, make sure "Inbox" is selected at the bottom, and click "Run Now". This should delete all the bugspam. You can then delete the filter.
