In IE, the yellow info bar is one big click target. When using Firefox, I've seen several users attempt to click the bar outside the button (on extension install in particular). I've found myself doing this too.
See bug 295335 - I don't know what the accessibility reasons were.
As Gavin mentioned, this was by design for accessibility reasons (see bug 295335). We're also starting to migrate away from the notification boxes, since (among other reasons) they're too easy for websites to spoof. Marking WONTFIX.
