The other day I ran checksetup.pl on a 2.8 installation, and when the upgrade finished, almost all of the users didn't have any email preferences set. That is, they had no entries in the email_setting table.
Okay, so the problem is that the emailflags column never exists, during the upgrade. Thus the code that inserts emailprefs for all users never runs.
Bug 547863 removed support for upgrades from Bugzilla 2.8.
