Go to Tools->Options->Tabs.
There is a setting "new page should be opened in" and You have two choices:
 - a new window
 - a new tab
select "a new window".
Now, go and start browse the web.
You will see that some sites do not open in new windows - they open in new tabs.
One sure place to check it is hotmail. Go to your inbox and open up an email with a link in it (forum topic notifications are good examples) and click the link... Tada...
I hate tabs, I want to be able to switch them off (once and for all), but it seems to be mission impossible.

Steps to Reproduce:
1. Select "a new window" in Tools->Options->Tabs
2. Go surf the net for a while (if it does not happen, just go to hotmail, it happens every time for me).


Actual Results:  
New pages open in new tabs.

Expected Results:  
I expected:
 - new pages to be opened _always_ in new windows,
 - get rid of those stupid, useless tabs (once and for all).

Not everyone is as keen on tabbed browsing as You think.
I for one believe it is the worst feature ever introduced. It causes a lot of headache to find the right tab over all the open windows.
You might want to add a single checkbox to the options that says "turn off all usage of tabs".
I marked it major because I think that the feature is broken (I set it to "new window" but it does not work) and it is essential part of browsing. Also, because the browser is a tool for me in everyday work.
Please let me know if this will be fixed any soon or not. I had quite enough trouble already so I am considering finding another browser.

Could you perhaps show a case outside of hotmail where it happens. Or add a testcase?

Tabs load a lot quicker is one of the big advantages.
And you can always place them on the bottom, above the taskbar buttons with some code or an extension.
But it needs time to get used to it and after that there's no return possible.
