The Apache logfile around that time shows: process_bug.cgi: Use of uninitialized value in concatenation (.) or string at Bugzilla/Mailer.pm line 90., referer: http://bugzilla.etc.to/show_bug.cgi?id=276 which seems to be related. Reproducible: Always Steps to Reproduce: 1. Add a comment 2. Submit. 3. Boom.

Sorry; I think this was caused by a typo in the host URL. The code derives a hostname from the URL using a regexp; my host URL was misspelled as http:/hostname/ instead of http://hostname/ causing the regexp to fail.
