The top border of a column in a border-collapsed table is not painted for columns that have no cell in their top row.

In the testcase, every cell has the contents "X", but rows have different numbers of cells.  There is a border-collapsed border on the columns.

Steps to reproduce:
 1. load attached testcase

Expected results: A complete border around all three columns

Actual results: There is no top border above the right column
