Please add the following rule to the netscaler caching for AMO:
If the cookie "AMOfbUser" exists, addons.mozilla.org/services/update.php should not be cached.
services/update.php is the actual filename, but I think clients use the rewritten addons.mozilla.org/update/VersionCheck.php
So maybe anything with "update" in it shouldn't be cached.

It looks like the rule wasn't added to the .nl netscalers - users there aren't able to auto-detect add-ons.
