 When starting my PC (Vista Home Premium) about 25% of the time firefox won't open. The odd thing is, the 75% of the time it does work, it doesn't quit working. So pretty much, when I boot the PC, theres a 25% chance firefox will not open unless I reboot. I have a friend who experiences the same problem. When I open firefox, I get a short load bar, but the browser never opens. It shows up on the task manager under the processes tab, but I can't end the process... it keeps running no matter how many times I click end process.

 Since Firefox can't be running in the background to run safe mode, I have not tried to open it. I never know when its not going to work so I never know when to try safe mode. As I said, the odd thing is if it opens after I boot it will open every time. If it doesn't open after boot, it will not open unless I reboot. I don't really use any extensions and almost every setting is on default anyway. Lets say I boot up my PC, log into the only account on my PC, which is my admin account, then the desktop loads. I click the Firefox icon on the desktop or under "all programs" and it doesn't open. I have to reboot and try again. After clicking the icon, though the browser doesn't open, its still running under "processes" and I can't end it. Firefox is my favorite browser though, so I'm trying to fix it =P
 
I have never seen this problem on Vista. Is firefox.exe always running after booting?
 After I boot, firefox.exe is NOT running. It only begins to run when I click the icon and it fails to open. Once it does begin though, it cannot be closed unless rebooted. As far as spyware/malware/trojan, its very unlikely. I use Windows Live Onecare and have used Symantec. Also, as a CIS major student its very unlikely I would have downloaded anything to contract the trojan, though possible. As I said, WLOC and Symantec didn't find any problems anyway. I also use Zone Alarm as a firewall, which I doubt is the problem, since I'm using firefox now and I always used Firefox and ZA with XP. Also, the symptoms:

#  Firewall alerts that Firefox is trying to connect to an unrequested IP address on a remote port (typically Port 3460, which is Poison Ivy's default Port setting - though any Port# may have been chosen).
# Firefox/system is mysteriously sluggish (The hacker may be searching for or transferring files, taking screencaptures, keylogging, packet-sniffing, etc).
# Unusual activity is noticed that seems to be being controlled by 'remote control' - such as unexpected mouse movement or perhaps character movement in online multiplayer games (The hacker is controlling the cursor). 

Are not occurring. 
 Just for clarity, when i said "since I'm using firefox now and I always used Firefox and ZA with XP", I meant before I upgraded to vista. This is a new PC with Vista Home Premium, 2 gigs ram, duo core processor, nvidia geforce 7600 if any of that helps. 
Normally you should be able to terminate firefox.exe. If that's not possible it sounds like the work of a firewall-like program which can catch the process and lock it up.

 Next time the program doesn't launch I'll try disabling ZoneAlarm to see if it allows me to close it then. It would also be important to bring up the fact that my friend is also experiencing the same problem. Thing is, they have ZA installed too, so I'll just have to wait and see if it works, though I have set Firefox as trusted. 
 I just booted my PC and when I tried to open Firefox it appeared in the processes tab, but did not open. Safe mode won't open due to the fact that firefox is "running". I can't close the process because when i click end process it doesn't do anything. I disable and closed my firewall and still could not exit the process. Its running right now as a matter of fact. Firefox.exe is 6,496 K in memory. 
 The odd thing is, now I have a message when i tried to terminate the process. "The operation could not be completed. Access is denied." I've check for viruses and Trojans once again, still have none. Also, I know more than one user with this problem.
To add to the comment I just made, Ive waited a while without rebooting and I tried to open Firefox again and it prompted me to start a new session or continue old one, I started a new session and its working now.. It takes like an hour though..
As a test you could try unzipping Firefox on your desktop and run it from there. If you can't use safe-mode you could simply drag the folder extensions out of your profile, although neither the extensions, themes and plugins nor Firefox itself have the power to keep firefox.exe running if you want to end the process.

And don't forget to temporarily disable updating in the Options window -> Advanced otherwise the firewall will see the updated application as a new program after every new automatic update.
I have been experiencing the same problem, as has my son on his PC. Both new machines running Vista (one Vista Home, the other Vista Home Premium - I HATE VISTA!). Didn't find this when I searched. 

I don't know if Bug 404287 should combined with this one or not.

When I was searching on this problem, I ran across some references to others with the same problem. These references were on non-Mozilla boards.
an easy way to tell if firefox shuts down all the way, in the firefox dir create a ff.bat file with
  .\firefox.exe
open a command window, navigate to the dir, type ff.  when you quit firefox if it shuts all the way then you'll see the results in the command window, it will be back at the DOS prompt.  But this won't of course tell you if all the files are in a correct state.

Have you narrowed down the issue to being FF or not FF?  i.e. turn off EVERYTHING on the PC that isn't vanilla VISTA OS (disconnect from the network if necessary).  If your failure is reproducible as you suggest you shouldn't have a problem shooting it.
