I don't see how people can stand the mindprint of all these ancient bits of cruft - nsMdbPtr.h was included in nsGlobalHistory.cpp in rev. 1.14, August 1999, then removed and orphaned in rev. 1.49, August 2000, so it was not quite two years dead when it was forked to browser/, but more than two years dead when it was moved to toolkit/.
