Details

I don't have a lot of details, but when someone does not have a previous account and logs into the system with LDAP, their account seems to be incomplete - it's missing an email address and the "use ldap" field isn't checked.
I added some code to disallow logging in unless you have an account already.  Logging in before an admin makes you an account is pointless anyway since you'll just be looking at an error suggesting you contact an admin to get permissions.
