WFM (I get 1 blank tab per double click on blank tab bar area) with 3beta2rc1 && 126.96.36.199 @ Mac 10.4.11
What are you double-clicking to get a new tab?
to answer your question Steve, I'm double-clicking within the empty space to the right of the default tab.
So you still see this problem if you make a new firefox profile for a test? - http://kb.mozillazine.org/Profile_manager If so, is your mouse button dodgy? Have you changed any OS settings with respect to how mouse clicks work or anything? You mention a viral bug... have you scanned your machine for viruses or other malware?
