Screenshot
bug 339804 and bug 348997 (are they dupes?) describe restoring a session with multiple tabs for the same site requiring authentication.
When this occurs I see (intermittently) an undersized prompt which is missing the descriptive text (see screenshot)

Same bug as bug 339804 comment #3. Marking depending and moving out of Firefox:Session Restore as this per bug 339804 #9 this isn't restricted to the use of Session Restore.

This seems to have been resolved by the fix to multiple prompts.
