All "exceptions" windows in the preferences have a close button except the passwords one, which is an accesibility problem. See screenshot

I can reproduce this on Linux, but not on Windows. Mozilla/5.0 (X11; U; Linux x86_64; en-US; rv:1.9b5pre) Gecko/2008030516 Minefield/3.0b5pre

The documentation at http://developer.mozilla.org/en/docs/Preferences_System:prefwindow says the attribute is dlgbuttons. Using this fixed it for me on Linux, but I can't test it on Windows at the moment.

Hmm, interesting that on OS X none of these windows (including the Preferences window itself) has a close button.

If you like seeing how the other half (the other third?) lives, just toggle browser.preferences.instantApply

Using Mozilla/5.0 (Macintosh; U; Intel Mac OS X 10.5; en-US; rv:1.9pre) Gecko/2008042904 Minefield/3.0pre, I noticed that the Addons/Exceptions window does not have a close button, but Windows and Linux do.
