I think the screenshot describes it better, but a small square is first shown before the menu (from the menu toolbar or from the bookmarks toolbar) shows up.

I'd say that the culprit is bug 405719, but it's just a guess.

as far as i can tell this only happens the first time the menu is displayed for its associated window.
