Fields are scrambled.

In the import dialog, did you correctly move the fields up and down so that they match?

Yes, the fields match up. The actual address book record does not match the imorted record, it appears the import template does not match the address book record fields.

Please could you attach an example of your imported file that exhibits the problem (doesn't have to be the real one as long as it shows the problem), and highlight/explain where it does not match the address book record fields once imported?

I've just tried this on the latest version of Thunderbird. I think it is actually your import file that is incorrect.
This is what I did:
1) Started Thunderbird & went to the address book
2) Selected Tools->Import
3) Selected the address book option, and next, then next again to accept the text file option.
4) Located the import file & selected it
5) Made sure the address book fields matched the record data to import.
6) Unchecked the "First record contains field names" option.
7) Selected OK to import the data.
So the address book I got contained two cards - as I had unchecked the option at item 6.
The first was a card with the field names in it, the values all matched the field names in your file AND were located in the correct fields as I'd matched it in the import dialog at step 5.
The second card looked incorrect, e.g. a phone number is in the secondary email address; custom 4 has an email address and a house address within it. Presumably this is the card you are asserting is being imported incorrectly.
However, if I check the matching of the field names in the first row of the csv file to the actual fields on the second line of the csv file, then I see that the second card has actually imported correctly according to the structure of the csv file.

You are correct, the import from (.csv) is very unforgiving. Thanks for your time!
